Social Prescribing

Our team of Social Prescribers are working with patients of Fitzalan Medical Group and The Coppice and Angmering GP surgeries to help improve your wellbeing by linking you to appropriate services and support.

We can help you find a new hobby, connect with others in your community or manage a health condition: whatever matters to you. We work with you in a series of sessions tailored to your needs, including attending sessions with you if you're anxious about going alone. Our team is not medically trained but can help you take control of your wellbeing and link in with your healthcare providers as needed.

Home Visiting

For those that can’t come into our centres, we offer a specialist service for residents in Arun and Chichester which helps with ongoing issues and visiting your home if needed. You can ask any professional via our third party referral form or self-refer via the channels mentioned below.
